To assist A/Prof Winder and Dr Lukins in their assessment please bring along the following:
- Referral letter from GP
- Medicare card, DVA card, Pension Card
- Have your Private Hospital Insurance information with you.
- Copies of pathology results or other investigations
- X-rays, MRI and CT Scans for viewing
- Any relevant reports
